3.4 Inspection and tagging

Access equipment fails on the day nobody looked at it. OSHA answers that with a web of inspection duties spread across several standards, each with a different frequency and a different person responsible. This short chapter pulls them into one place, then covers what happens to equipment that fails: how it gets tagged, who may return it to service, and what a crew should write down.

1 Who inspects what, and how often

Above all of the specific rules sits 29 CFR 1926.20(b)(2): the employer's accident prevention program shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer. The equipment-specific standards then set the cadence.

Two patterns are worth noticing. The higher the consequence of a silent failure, the more often the standard requires the look — personal fall arrest is the only item on the list inspected before every use, and it is the only one where you are the inspector. And nearly every entry has a second trigger: "after any occurrence." Equipment does not care about the calendar; it cares about what happened to it.

2 What "visible defects" means in practice

The standards use the phrase "visible defects" and leave the list to the competent person, except for ladders, where 1926.1053(b)(16) names them: broken or missing rungs, cleats or steps; broken or split rails; corroded components; and other faulty or defective components. Use that as the pattern for everything else — look for anything missing, broken, deformed, corroded, cut, or no longer doing its job.

For soft goods — harnesses, lanyards, slings — the checks are different because the damage is subtle. Bend webbing into an inverted U along its length and look for cuts, pulled or broken stitching, glazing from heat, chemical staining, and UV fading. Check that the impact indicator on a shock pack has not deployed. Confirm that the labels are still readable: a harness with no legible label cannot be shown to meet anything, which is the same problem as unmarked PPE from chapter 2.1.

3 Tagging out and removing from service

1926.1053(b)(16) is the clearest instruction in this area: a defective portable ladder shall either be immediately marked in a manner that readily identifies it as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired. Two verbs, both required — mark it and take it out of use.

Do it in that order, and physically. A tag on a ladder still leaning in the stairwell will be ignored by someone in a hurry. Take the item to a place where it cannot be picked up by accident, and tell the supervisor rather than assuming the tag speaks for itself.

    Repair or destroy. Anything that cannot be repaired to the manufacturer's instructions gets cut up so it cannot come back.

    Only the competent person returns it to service — and for fall protection that took an impact, that decision is required by 1926.502(d)(19).
Common mistake: the "bad ladder" everyone on the crew knows about and nobody tags, because it still works if you avoid the third rung. Institutional knowledge is not a control — the new hire does not have it.

4 Scaffold tags: practice, not regulation

Green, yellow and red scaffold tags are an industry convention, and a useful one, but Subpart L does not require them. What it requires is the competent person's inspection before each work shift under 1926.451(f)(3), guardrails installed before the scaffold is released for use by anyone other than the erection crew under 1926.451(g)(4)(i), and erection or alteration only under competent-person supervision under (f)(7).

Where tags are used, the usual meanings are: green for complete and ready to use as built; yellow for a scaffold with a known limitation — incomplete guardrails, restricted load, fall arrest required — where the tag states the limitation; and red or no tag for do not use. Treat a tag as information, never as a substitute for looking. A tag with an old date, an unsigned tag, or a green tag on a scaffold missing a guardrail all tell you to ask before you climb.

5 Writing it down

Subpart L and Subpart X do not require an inspection record — but the training standards do require records of training (1926.503(b)(1) for fall protection), and a written inspection is the only way an employer can later show that the shift inspection happened. Most contractors keep a simple daily log per scaffold or per crew, and it costs about two minutes.

Useful record, minimum content: what was inspected, the date and time, who inspected it (name, not initials of a company), what was found, what was done about it, and whether the equipment was released or removed. If a defect was found and corrected, the record should show both. Our pre-task plan has a place to note access equipment checks alongside the day's hazards, which keeps the whole thing on one sheet.

6 The same pattern in the rest of the gear

Once you know the pattern — a person, a frequency, a "remove from service" rule — you can read any equipment standard quickly. Two more that construction crews meet constantly:

Rigging. 29 CFR 1926.251(a)(1) requires rigging equipment for material handling to be inspected prior to use on each shift, and as necessary during use, with defective equipment removed from service. Slings get their own paragraph: under 1926.251(a)(6), each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer, with additional inspections during use where service conditions warrant, and damaged or defective slings removed from service immediately.

Fixed ladders. 1926.1053(b)(17) requires fixed ladders with structural defects — broken or missing rungs, cleats or steps, broken or split rails, corroded components — to be withdrawn from service until repaired, and 1926.1053(b)(18) requires any repair to restore the ladder to a condition meeting its original design criteria before it goes back into use. A field-welded rung on a caged access ladder rarely meets that test.

The through-line: the standard names who decides, how often they look, and what happens to the failed item. When you meet a new piece of equipment, ask those three questions and you will usually have found the rule.

7 Making it a habit

The inspection that actually happens is the one attached to something you already do. Tie the harness check to putting the harness on. Tie the ladder check to picking the ladder up. Tie the scaffold walk-around to arriving at the work area with your first load. Nobody sustains a separate inspection routine for long; everybody sustains a thirty-second habit attached to an existing motion.

And when you find something, say it out loud to the crew. Half of the value of an inspection is the next person hearing that the third plank is loose.
