1.4 The pre-shift examination and taking a truck out of service

1910.178(q)(7) is one of the shortest requirements in the standard and one of the most ignored: industrial trucks shall be examined before being placed in service, the examination shall be made at least daily, and where trucks run round the clock they shall be examined after each shift. Defects, when found, shall be immediately reported and corrected. Five minutes at the start of a shift is the entire ask.

1 What the standard actually requires

Three sentences carry the whole duty, and they are worth separating.

    Examine before service, at least daily. 1910.178(q)(7): trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle.

    After each shift on round-the-clock operations. Same paragraph. Three shifts means three examinations, not one at 6 a.m.
  3. 3
    Report and correct immediately. Same paragraph again — the examination is worth nothing if the finding dies in a clipboard.

Two neighboring paragraphs give the examination its teeth. 1910.178(p)(1): if at any time a truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. And 1910.178(q)(1): any power-operated industrial truck not in safe operating condition shall be removed from service, and all repairs shall be made by authorized personnel.

Note that (p)(1) says "at any time." The duty does not end when the inspection sheet is signed. A truck that develops a brake fault at 2 p.m. comes off the floor at 2 p.m.

2 What no paragraph says

1910.178 does not prescribe a checklist, a form, a retention period, or a signature. It requires the examination to happen and the defects to be reported and corrected. Everything else — the paper, the app, the tag system — is your employer's method for proving the first part happened.

That is not an argument for skipping the paperwork. Common mistake: pre-signing the week's sheets on Monday. It converts a safety control into a document, and it is the first thing an investigator notices after an incident.

3 The walk-around, engine off

Start cold and walk the truck. Everything in this list is visible without starting anything.

4 The operational check, engine running

Now start the truck, seat belt on, in a clear area where nothing is in front of the forks. You are testing the controls that will fail you at the worst possible moment.

  • Service brake and parking brake. Roll a few feet and stop. Set the park brake on a slight grade if you have one and confirm it holds.
  • Steering. Full lock both ways. Excessive free play, binding or a groan from the pump are all findings.
  • Lift, lower and tilt. Full travel, smooth, no drift. Raise the forks, hold, and watch for creep down — a leaking cylinder or valve will show itself in fifteen seconds.
  • Horn, lights and alarms. The horn is required equipment on a floor with pedestrians and it is the control you will use most under pressure.
  • Instruments and warning lamps. Hour meter running, no faults lit, fuel or battery state adequate for the shift.
  • Attachment function. Side shift, clamp open and close, rotator — each through its full range.
On the job: the fork drift test is the one most operators skip and the one most likely to catch a real fault. Raise the forks about four feet, mark the height against a rack upright with your eye, and look again after a slow count of fifteen.

5 Tagging out and who may repair

When something fails the examination, the truck comes off the floor. That means keys out and a physical tag on the controls saying the truck is out of service, in a place the next operator cannot miss. Parking it in the corner and telling the shift lead is not removal from service — the next person on shift does not know.

1910.178(q)(1) requires that all repairs be made by authorized personnel, and 1910.178(q)(2) prohibits repairs in Class I, II and III locations altogether. Two further limits are worth knowing because they get crossed by well-meaning mechanics: 1910.178(q)(3) requires that repairs to the fuel and ignition systems of industrial trucks which involve fire hazards be conducted only in locations designated for such repairs, and 1910.178(q)(4) requires that trucks in need of repairs to the electrical system have the battery disconnected prior to such repairs. 1910.178(q)(5) adds that replacement parts must be equivalent as to safety with those used in the original design.

Where the repair involves servicing that exposes someone to unexpected energization or stored energy — a mast raised on blocks, a hydraulic accumulator, an electrical system — the energy control requirements of 1910.147 apply to that work, and the technician doing it needs the lockout program, not just a tag on the seat.

6 Two more things the standard puts on the truck

1910.178(e)(1) requires that high lift rider trucks be fitted with an overhead guard manufactured in accordance with paragraph (a) of the section, unless operating conditions do not permit. That guard is not optional trim: it is what stands between an operator and a load falling off the top of a stack.

1910.178(e)(2) requires a vertical load backrest extension to be fitted where the type of load presents a hazard of falling rearward through the mast and onto the operator. If your loads are small cartons stacked to the top of the backrest, that is exactly the hazard the paragraph names.

Neither of those is a daily wear item, but both belong on the walk-around, because both get bent by rack impacts and quietly stay bent. A cracked weld on an overhead guard is a finding.
