Scaffold Safety

Hazards: Falls from the platform or during erection · Collapse from overloading or bad footing · Struck-by falling tools and material · Contact with overhead power lines

Why it matters

Scaffolding, 29 CFR 1926.451, was the sixth most cited OSHA standard in fiscal year 2025, and it is still killing people: scaffolds and staging were the primary source of 51 fatal falls, slips and trips in construction in 2022, up from 49 in 2021 (BLS Census of Fatal Occupational Injuries, in CPWR's March 2024 Data Bulletin). Almost none involve an exotic failure — a missing plank, a base plate on a block, a guardrail taken down "for a minute," a worker on the braces.

Key points

  • Fall protection above 10 feet — not 6 — and the system depends on the scaffold type. 1926.451(g)(1).
  • The competent person owns it. Inspection for visible defects before each work shift — (f)(3) — and no erecting, moving, altering or dismantling except under their direction — (f)(7). Pulling a plank counts.
  • Fully planked, gaps under 1 inch, 14 inches from the work. Platforms at least 18 inches wide, front edge within 14 inches of the work face unless guarded. (b)(1)-(b)(3).
  • Firm footing below, tied above. Base plates and mud sills on level, sound footing — (c)(2); guyed or braced above a 4:1 ratio — (c)(1); 4 times the intended load — (a)(1). Blocks are not a foundation.
  • Crossbraces are not a ladder. Safe access required above 2 feet, and (e)(1) says crossbraces shall not be used as a means of access.
  • Mind the wires and what drops. 10 feet from lines up to 50 kV, 3 feet from insulated lines under 300 V — (f)(6); below, hard hats plus toeboards, screens or guardrails — (h).

Do / Don't

Do

  • Check the tag and ask who inspected it this shift.
  • Walk the deck before you load it: planks, guardrails, midrails, toeboards.
  • Use the ladder or stair tower, both hands free, tools hoisted.
  • Report a missing plank or loose brace to the competent person right away.

Don't

  • Don't remove a guardrail, plank or tie — that is an alteration, and not yours to make.
  • Don't climb crossbraces, or stand on a bucket, box or ladder on the platform.
  • Don't shim legs with scrap, stockpile the deck, or work up there in high wind.

  • Nearest scaffold: fully planked, every gap under an inch?
  • The legs: base plates and mud sills on level, sound footing — or blocks?

Frequently asked questions

More than 10 feet above a lower level, under 29 CFR 1926.451(g)(1) — not 6 feet. The 6-foot rule in 1926.501(b)(1) is for other walking/working surfaces. Some scaffold types require fall protection regardless of the general rule: a boatswain's chair, catenary, float, needle beam or ladder jack scaffold requires a personal fall arrest system, and a single-point or two-point adjustable suspension scaffold requires both a personal fall arrest system and a guardrail.

1926.451(f)(3) requires a competent person to inspect the scaffold and its components for visible defects before each work shift, and again after any occurrence that could affect its structural integrity — a vehicle strike, a storm, a change in the load.

No. 1926.451(e)(1) requires safe access whenever the platform is more than 2 feet above or below an access point, and it states that crossbraces shall not be used as a means of access. Use a ladder, stair tower, ramp, integral prefabricated frame access, or direct access from another surface.

1926.451(b)(3) puts the front edge of the platform no more than 14 inches from the face of the work unless a guardrail is erected along the front edge or a personal fall arrest system is used. The limit is 18 inches for plastering and lathing, and 3 feet for outrigger scaffolds.

1926.451(f)(7) allows erection, moving, dismantling or alteration only under the supervision and direction of a competent person qualified in scaffold erection, using experienced and trained employees selected by that competent person. Training itself is required by 1926.454 — by a qualified person for users, by a competent person for erectors.
