How to use this checklist
- 1Name the scaffold, not the elevation. “The north scaffold” stops being a record the day a second one goes up. A scaffold ID on the sheet and on the tag is what lets anyone match this inspection to that structure a month later.
- 2Start at the mud sills and work up. Foundation, plumb and bracing, then platform, then access, then the rails. Almost every scaffold collapse investigation begins at the base — a sill that sank overnight, a leg off its base plate, a tie removed to let a material hoist through.
- 3Do the two conditional groups honestly. The mobile group and the suspension group are marked N/A on a static frame scaffold, and they are the whole inspection on a rolling tower or a swing stage. A blanket N/A down the page is how a caster lock gets missed.
- 4Write what you did about each defect, not just what you found. 1926.451(f)(4) gives four outcomes and no fifth: repaired, replaced, braced to meet the strength requirement, or removed from service until repaired. “Noted” is not one of them.
- 5Post the result and sign. Print or download the PDF. Check the access ladders with the ladder inspection checklist, the fall protection kit with the harness and lanyard form, and brief the crew from the scaffold safety toolbox talk.
What OSHA requires
One sentence carries the duty. 29 CFR 1926.451(f)(3): “Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold’s structural integrity.” Read it as two separate triggers. The first has a schedule — every shift, before anyone climbs. The second has none: a struck leg, a night of high wind, a load dropped on the deck or a tie pulled to let a hoist through all start the inspection over, whatever the clock says.
The inspection belongs to a competent person, defined at 1926.32(f) as someone capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. That second clause is the one that decides whether the role is real on a given site: an inspector who can write a finding but cannot stop the trade from climbing is not what the definition describes. 1926.454(b) then requires anyone who erects, disassembles, moves, operates, repairs, maintains or inspects a scaffold to be trained by a competent person for that work — while 1926.454(a) puts everyone who merely works on the scaffold in the hands of a person qualified in the subject matter. This form records an inspection. It does not qualify anyone as a competent person, and no form can.
What happens next is prescribed too, and narrowly. 1926.451(f)(4): any part damaged or weakened such that its strength is less than paragraph (a) requires shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired. Four outcomes, and “flagged for later” is not among them.

Two things this form deliberately does not do. It does not publish a scaffold tag colour code as if OSHA required one — Subpart L never mentions a tag, and green/yellow/red is an industry convention worth using and worth naming as a convention. And it does not stand in for a scaffold design: 1926.451(a)(6) requires the scaffold to be designed by a qualified person and constructed and loaded in accordance with that design, so the maximum intended load on this sheet is copied from that design, never guessed here. A scissor lift, incidentally, belongs on this form more than on an aerial lift form: OSHA’s scissor lift Hazard Alert lists 1926.451, 1926.452 and 1926.454 as the standards that apply to it.
