Scaffold Inspection Checklist Generator

Identify the scaffold, work through the criteria 29 CFR 1926.451 and 1926.452 set for its foundation, platform, access, guardrails and use, record the defects and sign the form, then print it or download it as a PDF. How to use this checklist

    Name the scaffold, not the elevation. “The north scaffold” stops being a record the day a second one goes up. A scaffold ID on the sheet and on the tag is what lets anyone match this inspection to that structure a month later.

    Start at the mud sills and work up. Foundation, plumb and bracing, then platform, then access, then the rails. Almost every scaffold collapse investigation begins at the base — a sill that sank overnight, a leg off its base plate, a tie removed to let a material hoist through.

    Do the two conditional groups honestly. The mobile group and the suspension group are marked N/A on a static frame scaffold, and they are the whole inspection on a rolling tower or a swing stage. A blanket N/A down the page is how a caster lock gets missed.

    Write what you did about each defect, not just what you found. 1926.451(f)(4) gives four outcomes and no fifth: repaired, replaced, braced to meet the strength requirement, or removed from service until repaired. “Noted” is not one of them.

    Post the result and sign. Print or download the PDF. Check the access ladders with the ladder inspection checklist, the fall protection kit with the harness and lanyard form, and brief the crew from the scaffold safety toolbox talk.

What OSHA requires

One sentence carries the duty. 29 CFR 1926.451(f)(3): “Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold’s structural integrity.” Read it as two separate triggers. The first has a schedule — every shift, before anyone climbs. The second has none: a struck leg, a night of high wind, a load dropped on the deck or a tie pulled to let a hoist through all start the inspection over, whatever the clock says.

The inspection belongs to a competent person, defined at 1926.32(f) as someone capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. That second clause is the one that decides whether the role is real on a given site: an inspector who can write a finding but cannot stop the trade from climbing is not what the definition describes. 1926.454(b) then requires anyone who erects, disassembles, moves, operates, repairs, maintains or inspects a scaffold to be trained by a competent person for that work — while 1926.454(a) puts everyone who merely works on the scaffold in the hands of a person qualified in the subject matter. This form records an inspection. It does not qualify anyone as a competent person, and no form can.

What happens next is prescribed too, and narrowly. 1926.451(f)(4): any part damaged or weakened such that its strength is less than paragraph (a) requires shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired. Four outcomes, and “flagged for later” is not among them.

Two things this form deliberately does not do. It does not publish a scaffold tag colour code as if OSHA required one — Subpart L never mentions a tag, and green/yellow/red is an industry convention worth using and worth naming as a convention. And it does not stand in for a scaffold design: 1926.451(a)(6) requires the scaffold to be designed by a qualified person and constructed and loaded in accordance with that design, so the maximum intended load on this sheet is copied from that design, never guessed here. A scissor lift, incidentally, belongs on this form more than on an aerial lift form: OSHA’s scissor lift Hazard Alert lists 1926.451, 1926.452 and 1926.454 as the standards that apply to it.

Frequently asked questions

A scaffold that sat untouched through a night of high wind is inspected again before anyone climbs it.

Someone the employer has designated, who meets the definition at 1926.32(f) — capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. The second half is the half sites forget: authority to stop the work and pull the tag is part of the definition. 1926.454(b) then requires that anyone who erects, disassembles, moves, operates, repairs, maintains or inspects a scaffold be trained by a competent person to recognize the hazards of that work. This form records an inspection; it does not qualify anyone, and no form can.

No. Green, yellow and red scaffold tags are an industry convention, not an OSHA requirement — Subpart L never mentions a tag. What the standard does require is the inspection itself, and that any part damaged or weakened such that its strength is less than 1926.451(a) demands be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired (1926.451(f)(4)). A tag is a good way to communicate the result of the inspection to the crew, which is why this form has a field for the tag status; it is not what makes the site compliant.

More than 10 feet above a lower level, under 1926.451(g)(1) — not the 6 ft of Subpart M. That single number is the most common confusion on a mixed site, and it only applies to work on a scaffold: an employee on a leading edge next to the scaffold is back under 1926.501(b)(1) at 6 ft. Paragraphs (g)(1)(i) through (vii) then say which protection each scaffold type needs; on a two-point adjustable suspension scaffold, for instance, it takes both a personal fall arrest system and a guardrail system.

Between 38 and 45 inches above the platform surface for supported scaffolds manufactured or placed in service after January 1, 2000. Supported scaffolds placed in service before that date, and all suspended scaffolds where both a guardrail and a personal fall arrest system are required, sit between 36 and 45 inches. That is 1926.451(g)(4)(ii), and it is not the 42 in ± 3 of Subpart M. The strength criteria are in (g)(4)(vii) and (ix): a toprail withstands 200 lb and a midrail 150 lb on most scaffolds, dropping to 100 lb and 75 lb on single-point and two-point adjustable suspension scaffolds.

Largely yes, and that surprises people. A scissor lift is not an aerial lift under 1926.453 — its platform goes straight up and down rather than being positioned on a boom — and OSHA's own scissor lift Hazard Alert lists 1926.451, 1926.452 and 1926.454 as the standards employers must comply with. So the guardrail, capacity, stability and training criteria on this form apply. The daily control test and the machine-specific checks belong on the aerial lift and scissor lift form instead; the two are meant to be used together.

No, and the standard says it in one line. 1926.451(e)(1) ends with: crossbraces shall not be used as a means of access. Access has to come from a portable, hook-on or attachable ladder, a stair tower, a stairway-type ladder, a ramp, a walkway, integral prefabricated scaffold access, or direct access from another scaffold or structure — and it is required whenever the platform is more than 2 feet above or below the point of access. Where crossbracing is allowed is as a rail: (g)(4)(xv) accepts it as a midrail when the crossing point is 20 to 30 in above the platform, or as a toprail when it is 38 to 48 in.

No. 1926.451(f)(3) requires the inspection; it prescribes no form, no format and no retention period, and Subpart L has no recordkeeping paragraph. A written sheet is simply how an employer shows the inspection happened, who made it, and what was done about what was found — and on a job where three crews and two subcontractors use the same tower, a signed sheet with the scaffold ID on it is the only version anyone can check afterwards.
