Stepladder Misuse

Why it matters

Falls to a lower level were the most common fatal event in construction in 2022 — 36.4%, 397 deaths (CPWR Data Bulletin, July 2024, from BLS CFOI). Stepladders put you five feet up, which does not feel like height. On December 26, 2022, a worker taking a measurement for a garage beam was found on the slab, unresponsive; the 8-foot fiberglass stepladder lay next to him with its metal spreader arms slightly folded up and not locked. He died of a skull fracture (OSHA Accident Summary Nr 153239.015). Five feet onto concrete is enough.

Key points

  • The top and the top step are not steps. 1926.1053(b)(13). General industry adds the cap — 1910.23(c)(8). If the work is out of reach, the ladder is too short.
  • Spreaders locked, both sides, every time. (a)(8) requires a metal spreader or locking device holding the front and back sections open while the ladder is in use. Push them flat and hear them seat before your weight goes on.
  • A folded stepladder is the wrong ladder. (b)(4): used only for the purpose it was designed for. Leaned against a wall it has no rung locks, no angle and no even bearing.
  • All four feet, one stable level surface. (b)(6). Never on a box, a barrel, a pallet or a scaffold platform to gain height — 1910.23(c)(13).
  • Don't climb the back. (b)(14): rear cross-bracing is not for climbing unless the ladder is built with steps on both sections.
  • Keep your belt buckle between the rails. Reaching sideways is what tips a stepladder. Move the ladder — it takes ten seconds.
  • Nothing in your arms going up. (b)(22): no object or load that could cost you your balance. Tool belt, bucket, hand line.
  • The "top three rungs" is not an OSHA rule. For a stepladder, OSHA's line is the top and the top step. The three-rung mark on a straight ladder comes from the manufacturer and ANSI A14 — obey the label on your ladder, but do not quote it as federal law.

  • Set one on a pallet, a box or a scaffold deck.
