5.4 Fall zone, swing radius and landing the load
Rigging failures hurt people because people are underneath. Everything else in this course reduces the chance of a failure; the zones reduce what happens when one occurs anyway. They are also the part of a lift that costs nothing, needs no calculation, and is abandoned first when a job gets busy.
1 The fall zone
The rule is short. 1926.1425(b): while the operator is not moving a suspended load, no employee may be within the fall zone, except employees engaged in hooking, unhooking or guiding a load; engaged in the initial attachment of the load to a component or structure; or operating a concrete hopper or concrete bucket.
Three narrow exceptions, all of them people who have to be there to do the job. Everyone else is out. And 1926.1425(a) adds a duty about the route itself: where available, hoisting routes that minimise the exposure of employees to hoisted loads must be used, to the extent consistent with public safety. The best exclusion zone is a path that never goes over anyone.
For the people who are allowed inside, 1926.1425(c) imposes three conditions at once: the materials must be rigged to prevent unintentional displacement; hooks with self-closing latches or their equivalent must be used, with a narrow exception for J hooks setting wooden trusses; and the materials must be rigged by a qualified rigger. That last one is the reason Part 5's roles chapter matters — the qualified rigger requirement is triggered by people being close to the load, not by the size of the lift.
One more, for landing: 1926.1425(d) — only employees needed to receive a load are permitted within the fall zone when a load is being landed.
2 How big is the fall zone, really?
Bigger than the shadow of the load. A load that comes loose does not drop straight down: it is moving with the boom, it can rotate, it can bounce, and long items pivot as they go. Slings that part release stored energy. Anything on top of the load — dunnage, banding, tools, ice — separates and travels further than the load itself.
Two practical rules that no standard prescribes but every experienced crew uses: set the boundary from the largest dimension of the load, not from its footprint, and be generous in the direction the load is travelling. Then keep the zone moving with the load rather than marking it once at the pick point.
3 The swing radius: a second, different zone
The fall zone is about the load. The swing radius is about the machine, and it kills people who are nowhere near the load. 1926.1424(a)(1) applies wherever there are accessible areas in which the rotating superstructure poses a reasonably foreseeable risk of striking and injuring an employee, or pinching or crushing an employee against another part of the equipment or another object.
Where that risk exists, (a)(2) requires the employer to do two things:
- 1Train each employee assigned to work on or near the equipment — "authorized personnel" — in how to recognise struck-by and pinch or crush hazard areas posed by the rotating superstructure.
- 2Erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas. Only where the employer can demonstrate that barriers are feasible neither on the ground nor on the equipment may the areas instead be marked by a combination of warning signs — the standard's own example is "Danger—Swing/Crush Zone" — and high visibility markings on the equipment, with each employee trained to understand what the markings mean.
Note the order: barriers are the requirement and signs are the exception you have to justify. It is the reverse of what most sites do.
4 Going somewhere the operator cannot see you
1926.1424(a)(3) contains a two-way protocol that is worth memorising, because it is the specific answer to the most common swing-radius fatality.
- (a)(3)(i) — before an employee goes to a location in the hazard area that is out of view of the operator, the employee, or someone the employee instructs, must ensure that the operator is informed that they are going there.
- (a)(3)(ii) — where the operator knows an employee went to such a location, the operator must not rotate the superstructure until informed, in accordance with a pre-arranged system of communication, that the employee is in a safe position.
Two things follow. The pre-arranged system has to be arranged in advance, which is a briefing item and not an improvisation. And the obligation is symmetrical: the person going in has to say so, and the operator has to wait for the confirmation, not for a guess about how long it usually takes.
5 Controlling the load without touching it
Hands on a suspended load is how people end up in the fall zone at the worst moment. Tag lines are the alternative, and they are a skill rather than a rope.
- Long enough to stand clear. A short tag line puts you back under the load.
- Attached low, so the pull controls rotation rather than tipping the load.
- Never wrapped around a hand, wrist or body. A load that swings takes the person with it.
- Used to damp, not to steer. Fighting a swinging load with a tag line adds energy; resisting gently removes it.
- Two lines on anything that wants to rotate, and a plan for who holds which.
- Released before landing, so nobody is tethered to a load that shifts as it touches down.
And the sequence at the set point: land the load, get it stable and supported, then approach. Blocking or cribbing goes in before hands do, because the last inch of a lift is when a load is most likely to shift — and because 1910.184(c)(12) prohibits pulling a sling from under a load resting on it, which means the blocking has to be planned before the load is on the ground, not after.
6 Setting the zones, in practice
- 1Walk the route on the ground first and look up. Live process, occupied areas, roads, doorways people come out of.
- 2Mark the swing radius with barriers, and only fall back on signs and high-visibility markings if you can show barriers are not feasible.
- 3Mark the fall zone at the pick, the path and the set — all three, not just the two ends.
- 4Tell the people who are not on the lift. Neighbouring trades, drivers, anyone whose route crosses yours. Most people in a zone are there because nobody told them there was one.
- 5Name who may enter, and confirm they are the ones the exceptions in 1926.1425(b) actually cover.
- 6Post someone to watch the boundary on a busy site. A line on the ground stops people who know what it means and nobody else.
That is the last item in the course, and it is deliberately the least technical. All the arithmetic in Parts 1 to 4 reduces the probability of a failure. The zones decide what a failure costs.
- 1926.1425(b): nobody in the fall zone under a suspended load except those hooking, unhooking, guiding, making the initial attachment, or operating a concrete hopper or bucket.
- Those who are inside it need the load rigged against unintentional displacement, self-closing hook latches, and rigging done by a qualified rigger — 1926.1425(c).
- Routes must minimise exposure of employees to hoisted loads where available — the best zone is a path over nobody.
- The swing radius is a separate hazard: barriers are the rule, signs plus high-visibility markings only where barriers are demonstrably infeasible — 1926.1424(a)(2).
- Before entering a hazard area out of the operator's view, tell the operator; the superstructure does not rotate until a pre-arranged confirmation says you are safe.
- Tag lines long, low, never wrapped around a hand, used to damp rather than steer, and released before landing.
