1.4 Holes, covers and falling objects

A large share of fall deaths happen to people who were not working at an edge. They were walking across a deck, or crouching to run conduit, and a sheet of plywood over a plumbing penetration slid, or a skylight panel gave way. And the mirror image kills people on the ground: a wrench, a brick, a bundle of strapping leaves a deck 40 feet up. Subpart M treats openings and falling objects as duties in their own right, with their own criteria, and this chapter covers both.

1 A hole is three separate duties

"Hole" is defined in 1926.500(b) as a gap or void 2 inches or more in its least dimension in a floor, roof or other walking/working surface. Note how small that is: 2 inches. And note that 1926.501(b)(4) imposes three distinct obligations, at three different triggers.

So a 4-inch core hole in a slab with an occupied floor below is a covered hole even though nobody can fall through it — (b)(4)(iii) is about the drill bit and the bolt that will go down it. And an 18-inch mechanical opening in a slab-on-grade, with no lower level at all, still needs a cover under (b)(4)(ii), because the hazard is a broken ankle and a torn knee.

On the job: the cheapest life-saving act on a deck takes ninety seconds — screw the cover down, spray it orange, write HOLE on it. Do it as the hole is created, by the crew that created it, not on a punch walk three days later.

2 Skylights

1926.501(b)(4)(i) says "holes (including skylights)" and it means it. A skylight is not a walking surface, and a skylight panel — plastic, decades old, brittle from ultraviolet exposure, sometimes painted over so it reads as opaque roof — will not hold a person. Workers die every year stepping onto one while carrying material, or sitting on the curb to rest.

There are three defensible answers, and only three: a screen or cover over the skylight that meets the cover criteria below, a guardrail system around it, or personal fall arrest for anyone who could reach it. A factory-installed dome that the manufacturer never rated as fall protection is none of those.

On a re-roofing job, count the skylights before the crew goes up and treat them as part of the layout, the same way you would treat a leading edge. They are usually in the middle of the roof, which is exactly where a warning line system leaves people unprotected.

3 What makes a cover a cover

The criteria are in 1926.502(i), and they are short enough to memorize.

    Roadways and vehicular aisles — the cover must support, without failure, at least twice the maximum axle load of the largest vehicle expected to cross it. 1926.502(i)(1).

    Everywhere else — at least twice the weight of the employees, equipment and materials that may be imposed on the cover at any one time. 1926.502(i)(2).

    Secured when installed, so as to prevent accidental displacement by wind, equipment or employees. 1926.502(i)(3). Screwed, cleated, or fitted into a recess — not laid loose.

    Marked — color coded, or marked with the word "HOLE" or "COVER" to provide warning of the hazard. 1926.502(i)(4). The standard does not require both; it requires one.

Note that the strength criterion is written as a multiple of the actual expected load, not as an absolute number. Nothing in the standard says "three-quarter-inch plywood." A sheet of half-inch OSB spanning 30 inches may satisfy (i)(2) for foot traffic and fail badly the day a pallet jack crosses it. If a vehicle will ever cross the cover, you are in (i)(1), and twice the axle load of a loaded forklift is a structural question, not a carpentry one.

Common mistake: marking the cover and forgetting to secure it — or securing it and forgetting to mark it. The two failures produce different accidents. An unsecured marked cover slides when someone steps on the edge. A secured unmarked cover gets lifted by the next trade who needs to run a pipe and never gets replaced, because nobody knew it was fall protection.

4 Falling objects: the hard hat is never the only answer

1926.501(c) is one sentence and a list, and it is written to close a specific argument. When an employee is exposed to falling objects, the employer shall have each employee wear a hard hat and shall implement one of the following:

  • Erect toeboards, screens or guardrail systems to prevent objects from falling from higher levels; or
  • Erect a canopy structure and keep potential fall objects far enough from the edge of the higher level that they would not go over if accidentally displaced; or
  • Barricade the area to which objects could fall, prohibit employees from entering the barricaded area, and keep objects far enough from the edge that they would not go over if accidentally displaced.

The hard hat is a floor, not a control. Read the list again and notice that two of the three options include keeping material back from the edge. That is the actual engineering answer to falling objects: material that is not near an edge does not fall off one. A canopy or a barricade with a stack of block sitting on the parapet above it satisfies nothing.

1926.501(c) also has to be read together with the hole duties: (b)(4)(iii) requires covers so objects do not fall through openings, and 1926.451(h) imposes a parallel duty on scaffolds.

5 Toeboards, screens and canopies — the numbers

1926.502(j) sets the criteria. A toeboard is not a piece of trim; it has a height, a strength and a maximum opening.

Two storage limits go with them. Under 1926.502(j)(3), in overhand bricklaying no materials or equipment except masonry and mortar may be stored within 4 feet of the working edge. Under (j)(4), materials and equipment may not be stored within 6 feet of a roof edge unless guardrails are erected at the edge — and where guardrails are used and materials are piled higher than the top edge of the toeboard, paneling or screening must extend from the walking surface to the top rail for a distance sufficient to protect employees below.

Canopies, under (j)(5), must be strong enough to prevent collapse and to prevent penetration by any objects which may fall onto them. A plastic tarp on pipe scaffold is a rain cover, not a canopy.

6 A deck routine that actually holds

The reason openings and dropped objects persist on well-run jobs is that both are created continuously by every trade and owned by none. The fix is procedural, and it belongs in the pre-task plan rather than in a monthly inspection.

    Whoever makes the hole covers the hole, the same shift, to the 1926.502(i) criteria — secured and marked. This is a rule about the person with the saw, not the person with the clipboard.

    Whoever lifts a cover replaces it, or guards the opening until it is replaced. No exceptions for "I'll be right back."

    Log the openings. A one-page list of every opening on every level, with who owns it and whether it is covered or guarded, turns a hunt into a check.

    Sweep the edges at the end of every shift. Tools, offcuts, fasteners and strapping within 6 feet of an edge go into a bucket or a bin, not into tomorrow's incident report.

    Barricade below, not above. When overhead work starts, the exclusion zone on the ground goes up first — hard barricade, signs, and someone responsible for keeping it clear.

    Tether what you carry. OSHA does not require tool lanyards, but a dropped-object program is the only measure that addresses the hand tool a toeboard cannot stop.
