Fall Protection Basics

Why it matters

Fall protection, 29 CFR 1926.501, was the most frequently cited OSHA standard in fiscal year 2025; fall protection training, 1926.503, came seventh. Nobody here intends to go over an edge. Falls happen in the ten minutes when the rail is off, the anchor is out of reach and the job is nearly done.

Key points

  • Six feet is the line. 1926.501(b)(1): protection at 6 ft or more above a lower level. Scaffolds are more than 10 ft, steel erection more than 15 ft — and below 6 ft over dangerous equipment you still need a rail, (b)(8).
  • Guardrails first, harness last. A rail protects everyone who walks past it without anyone thinking about it. Top edge 42 in plus or minus 3, midrail, toeboard where things can fall — 1926.502(b).
  • A, B, C. Anchorage at 5,000 lb per person, independent of any platform support (d)(15). Body support: a full body harness, never a body belt. Connectors: self-closing, self-locking, sized so they cannot roll out (d)(5).
  • Look down before you tie off. No free fall over 6 ft, no contact with any lower level, deceleration under 3.5 ft (d)(16). Add your height and the harness stretch: a 6 ft lanyard at deck level does not work 12 ft above concrete.
  • Inspect before every use. 1926.502(d)(21): for wear, damage and other deterioration; defective components come out of service. Fold the webbing into an inverted V along its whole length — cuts and broken stitches show in the bend, not flat.
  • Anything that took a fall is done. Out of service immediately, back only if a competent person says it is undamaged and suitable for reuse (d)(19). Most manufacturers say never.
  • Have a rescue, not a phone number. (d)(20) requires prompt rescue or self-rescue. Say out loud today who does it and with what.

Do

  • Check the harness before you put it on, every shift.
  • Anchor overhead and as close to plumb as you can.
  • Adjust the straps so a flat hand fits, no more.
  • Say something when you see an open edge, even for five minutes.

Don't

  • Tie off to a guardrail, a conduit, a duct or a ladder.
  • Clip a hook back onto its own lanyard webbing to tie off short.
  • Remove a rail or a hole cover and walk away from it.
  • Put a harness back in the bag after it stopped a fall.

Discussion — ask the crew
  1. Walk the deck we are on: where is the first unprotected edge, and what is protecting it right now?
  2. Point at the anchor you would tie off to for today's task. How do we know it holds 5,000 pounds?
  3. How far is it to the next level below? Is there enough room for the lanyard we are carrying to do its job?
  4. If somebody goes over that edge in the next hour and is hanging there, who gets them down, and how long does it take?
  5. Has anybody here got a harness in their gang box that took a fall or a drop and went back in the bag?

Frequently asked questions

6 feet or more above a lower level, under 29 CFR 1926.501(b)(1). General industry is 4 feet (1910.28(b)(1)(i)), scaffolds are more than 10 feet (1926.451(g)(1)) and steel erection is more than 15 feet (1926.760(a)(1)).

At least 5,000 pounds per employee attached, and independent of any anchorage used to support or suspend platforms — 1926.502(d)(15). The alternative is a system designed, installed and used under the supervision of a qualified person with a safety factor of at least two.

Not for fall arrest. 1926.502(d) has stated since January 1, 1998 that body belts are not acceptable as part of a personal fall arrest system. A full body harness is the only body support that arrests a fall.

The system must be rigged so you can neither free fall more than 6 feet nor contact any lower level, and it must limit deceleration distance to 3.5 feet — 1926.502(d)(16)(iii) and (iv). Add your height and the harness stretch and you see why the manufacturer's fall clearance chart matters as much as the anchor.

It comes out of service immediately and stays out until a competent person determines it is undamaged and suitable for reuse — 1926.502(d)(19). Most manufacturers do not allow reuse at all, and their instructions are the stricter rule.

1926.502(d)(20) requires the employer to provide for prompt rescue of employees in the event of a fall, or to assure that employees are able to rescue themselves. 'Call 911' is a phone number, not a plan — somebody has to be able to reach a suspended worker quickly.
