How to use this form

    Inspect before the crew goes in. 1926.651(k)(1) puts the first inspection prior to the start of work — not after the first load of pipe is set. Walk the whole length of the excavation and the ground behind the edge, not just the section being worked.

    Mark each checkpoint OK, Deficiency or N/A. Anything you leave blank prints as three empty boxes, so the form doubles as a clipboard sheet you fill out by hand.

    Pair every deficiency with an action. The corrective action table has a column for whether employees were removed from the hazardous area — that is the requirement in 1926.651(k)(2), and it is the line that matters most after an incident.

    Re-inspect after rain and after anything that changes the hazard. The third row of the inspection table exists for that. A trench that was fine at 7 AM is a different trench after an inch of rain.

What OSHA requires

29 CFR 1926.651(k)(1) is short, and every word of it drives the form above: “Daily inspections of excavations, the adjacent areas, and protective systems shall be made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection shall be conducted by the competent person prior to the start of work and as needed throughout the shift. Inspections shall also be made after every rainstorm or other hazard increasing occurrence. These inspections are only required when employee exposure can be reasonably anticipated.”

The standard requires the inspection, not a particular form, and it sets no retention period. Keeping the record is a practical and contractual matter — many owners and general contractors require a daily form, and State Plan states may impose requirements beyond the federal standard.

1926.651(k)(1) names rainstorms explicitly and then covers any other hazard-increasing occurrence. In the field that means heavy rain or snowmelt, freeze-thaw, a rise in the water table, vibration from traffic, pile driving or compaction near the cut, a load placed near the edge, a change in the soil that shows up as the excavation deepens, damage to a shield or shore, or a nearby excavation or blasting operation. Each of those calls for a new inspection before anyone re-enters.

Where the competent person finds evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees must be removed from the hazardous area until the necessary precautions have been taken to ensure their safety (29 CFR 1926.651(k)(2)). That is why this form pairs every deficiency with a corrective action and a line recording whether employees were pulled out.

One who is capable of identifying existing and predictable hazards in the surroundings, or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them (29 CFR 1926.650(b)). Both halves matter: the knowledge to classify soil under Appendix A and to judge a protective system, and the actual authority to stop the work and pull the crew out.

Subpart P Appendix B uses the term distress for soil in a condition where a cave-in is imminent or likely, and names the phenomena that show it: fissures in the face of or adjacent to an open excavation, subsidence of the edge, slumping of material from the face, bulging or heaving of material from the bottom, spalling of the face, and ravelling — small amounts of material separating from the face and trickling down. This form lists each of them as its own checkpoint so none gets skimmed over.
